Why they ran it

Seventy three healthy sedentary adults enrolled and chose whether to join a non exercise arm or an exercise and diet arm. Within that choice they were randomised, double blind and counterbalanced, to ten grams a day of creatine monohydrate in two doses or a maltodextrin placebo. Sixty four completed the twelve weeks. Body composition was measured by DXA, with fasting bloods and a battery of cognitive tests at baseline, six weeks and twelve weeks.

Without any exercise or diet change, creatine increased lean tissue mass, strength and muscular endurance, and moved selected blood lipids, HbA1c and some markers of cognitive function and memory in a favourable direction. Taken during the exercise and weight loss programme, creatine again increased lean tissue mass, produced a greater reduction in body fat percentage, and improved strength and endurance. Supplementation was well tolerated throughout.

The authors then do something worth noticing. They state that the individual cognitive and biomarker findings should be interpreted as exploratory. That is not a hedge added by a reader, it is in the paper, and it is the difference between a muscle result this trial was built to detect and a brain result it was not.

What this does not show

  • This does not establish that creatine improves memory or thinking. The authors explicitly label the individual cognitive and biomarker findings exploratory. A battery of tests with no single pre specified primary cognitive outcome will produce some movement by chance, which is exactly why that label is there.
  • The two settings cannot be cleanly compared. Participants chose whether to join the exercise arm rather than being randomly assigned to it. Randomisation applied only to creatine versus placebo within each group, so differences between the settings carry whatever made people choose.
  • Nothing here speaks to athletes or endurance performance. The participants were healthy but sedentary adults aged 45 to 65, and the measures were body composition, strength and muscular endurance rather than any endurance performance outcome.
